IMMIGRATION MEDICAL CLINIC in Burnsville, Minnesota conducts I-693 medical exams for green card applicants. Q. What should I bring with me to the appointment?


1. After getting Adobe Acrobat (latest edition), go to the USCIS site and click on I–693. Complete the first page and type your name on the top of the remaining pages. Then print the 13 page document and bring it with you.

2. Proof of vaccination, if available. Usually you will need T-dap within 10 years and MMR. If you did not have chickenpox as a child, Varicella will also be required. We are in the process of getting these vaccines and may be able to provide them in the future.

3. A Government issued photo ID such as a passport or driver’s license.

4. Any relevant medical records may be helpful.


Q. How much will it cost?


We try to keep the costs down as much as possible. The history and physical examination with completion of the 13 page document by the Civil Surgeon is not covered by insurance. The screening tests and immunization may be covered by your insurance. However, you will find our pricing fair and competitive. If you have a question, please feel free to call.


Q. What should I wear for the physical? Will I need to undress?


We suggest wearing loose clothing. No, you will not need to undress.


Q. How long will it take to get my appointment?


Fairly quickly. We can see you on the same day. We also make appointments after hours and on weekends to meet your needs. We understand that many of our patients come from far away and traffic and road construction are not always predictable. No worries, just call.


Q. How long will it take to see the Civil Surgeon?


We strive to make this a pleasant experience. After doing some paperwork you will be seen by the Civil Surgeon for the History and Physical. We will help you with all your questions and after the lab results are reported in a day or two, we will complete your I-693 and hand you a sealed envelope to be opened by USCIS only.


Q. Can I get an expedited I-693 sealed envelope?
